We accept referrals from all organizations in Mendocino County that work with households that are homeless or at-risk of homelessness.

Our Project Homekey funding requires us to provide transitional and/or permanent housing to households experiencing homelessness or at risk of homelessness. For this reason, referrals expire after 90 days.

If you would like a household to remain on our Interest List, please submit a new referral with a new Verification of Housing Status Form every 90 days.

Referral Forms

A complete referral for residency must include both the Referral Form and a Verification of Housing Status Form. Please include ALL potential members of the household on the referral form. Please also remember to indicate pet ownership and breed.

Incomplete referrals will not be accepted.

Animals at Live Oak

We have established both pet and companion or assistance animal rules to ensure the community is maintained in a decent, safe and sanitary manner and that all residents are allowed to live in peace and quiet comfort.

All animals, including companion or assistance animals, must be approved and registered before the potential resident is allowed to move-in.
